Interesting.  Can you give an example of where you are trying to add the second address?  I would like to duplicate the problem.

sorry for the late reply - I was away
here is a print-screen of the location for this problem
when I click the "Add" button nothing happens - same thing when I hit "Save changes"

ah yes, now I see. When you click on your photo in the top right corner of http://vision.mindje.com and then click on your own name, you'll end up in the screen above.

However, as Yves pointed out the button is broken. See debugging information below which will show up when pressing the Add button:
